The Place of the Khutba Adhan on Friday

Question

(1) The second adhan of Jumu‘a, which is given in front of the minbar — in the time of the Messenger of Allah ﷺ was it given inside the mosque or outside? (2) Where was it given in the time of the Rightly Guided Caliphs ﵃? (3) Have the reliable books of the Hanafi jurists forbidden giving the adhan inside the mosque and written that it is makruh, or not? (4)
