One prostration is not sufficient for sajda al-sahw
Question
The imam made a mistake and sajda al-sahw became wajib. For example, in a four-rak‘a prayer the imam by mistake gave the salam to one side after two rak‘as; the followers prompted him, and then the imam stood up for the third rak‘a. In the final sitting, performing sajda al-sahw, he performed one of the two prostrations, recited al-tahiyyat and the salawat and gave the salam. The followers said: “Respected imam, sajda al-sahw consists of two prostrations, but you performed only one.
Answer
Such a prayer should be prayed again: when sajda al-sahw becomes wajib because of omitting a wajib, it consists of two prostrations; one prostration is not sufficient. [ar]يَجِبُ بَعْدَ السَّلَامِ سَجْدَتَانِ بِتَشَهُّدٍ وَتَسْلِيمٍ بِتَرْكِ وَاجِبٍ[/ar] “For omitting a wajib, two prostrations with tashahhud and salam are wajib after the salam.
