Who Is a Mujaddid?

Question

What is the definition of a mujaddid (renewer of the religion)? Is it necessary for a mujaddid to appear at the beginning of every hijri century, or within the whole century? And if someone does not accept the mujaddid of his time, will he die the death of jahiliyya (ignorance)? How is a mujaddid recognised? Please write down the names of the mujaddids who have come over the thirteen hijri centuries.
